How we can help
Community Living Well offers a range of support for your social wellbeing, physical and mental health needs. We try to make it as easy as possible for you and other professionals to access our services by using a single contact point to refer and there is also the option to refer yourself. Our teams also work together in our service hubs at St Charles Health and Wellbeing Centre, Violet Melchett Centre and Gertrude Street, so it is possible for you to see the different people supporting you in one place.
To ensure that your care is right for you, the Community Living Well team may need to access and record information about you. This coordinated support means you do not have to repeat your story to different people in the service.
All personal information we hold about you is stored securely and in line with the Data Protection Act. Anonymised data may also be used to monitor the quality of the Community Living Well service.

Talking therapy or counselling that can help you if you are feeling stressed, worried, anxious or low in mood
Mother Tongue Counselling
One-to-one counselling in Arabic and Farsi
Primary Care Liaison Team
Specialist nurse-led support and access to Primary Care Psychiatry
Advice and guidance on a range of issues including benefits, debt, housing and signposting to specialist support services
Groups, social events and activities bringing people together to support and learn from each other
Advice and guidance on looking for work, staying in work, and improving employability skills
